Welcome to the 暗网吃瓜 family! We are thrilled that your student will be joining us and beginning an exciting new chapter in their academic journey. As we prepare for their arrival on campus, we want to make sure they are ready鈥�and that includes completing their Health Record Requirements.

Introduction

To ensure your student is cleared to start classes and participate in campus life, 颈迟鈥檚 essential that they complete all required health forms before arriving on campus. Please help your student schedule an appointment with their primary care provider as soon as possible to complete the following:听

• Physical Exam 鈥� Must be dated within one year of the start of classes
• Immunization Verification Form 鈥� Signed and dated by your student鈥檚 healthcare provider听

All forms and documentation must be uploaded through the Student Medicat Portal鈥�no health information will be accepted via email or fax.听

Accessing the Medicat Portal

Starting June 1, 2025, your student can access the Medicat Student Portal by visiting www.rivier.edu. Use the Quick Links dropdown menu and select Campus Groups to get started.听

Health Record Checklist听

For ALL Undergraduate Students (Resident and Commuter):听

• Upload completed Immunization Verification Form听听

• Manually enter each immunization date in the “IMMUNIZATIONS” tab
  • Click “All REQUIRED,” select each immunization from the list, and input the correct administration dates听

• Complete the TB Screening Form located in the portal听

For RESIDENT Students (in addition to the above):听

Complete and sign:听

• Informed Consent Form听

• Health History Form听

• Health Insurance Information Form听

Upload:听

• Physical Exam Form (signed and dated)听

• Front and back copy of your Health Insurance Card

Access all forms using the 鈥淓lectronic & Downloadable Forms鈥� tab in the portal.听

Special Notes for Student-Athletes听

Student-athletes must upload records to both SWOL and the Medicat Student Portal. The requirements differ, so be sure to complete all necessary documentation for each.听
